Abstract

Abstrak: Psoriasis merupakan penyakit inflamasi kulit kronis yang dipengaruhi oleh multifaktor dengan prevalensi di dunia berkisar 2-3%. TGF-β1 adalah faktor pertumbuhan yang mempunyai polimorfisme pada gennya. Polimorfisme gen TGF-β1 berperan dalam patogenesis psoriasis. Metode: Penelitian ini merupakan penelitian deskriptif dengan teknik simple random sampling. Subyek penelitian ini adalah bahan baku tersimpan (BBT) berupa hasil isolasis DNA penderita psoriasis sebanyak 62 sampel yang terdiri dari 33 perempuan dan 29 laki-laki. Penelitian ini bertujuan untuk melihat distribusi frekuensi polimorfisme gen TGF-β1 (T869C) pada penderita psoriasis. Polimorfisme gen TGF-β1 (T869C) dianalisa dengan PCR-based restricted fragment length polymorphism (PCR-RFLP). Hasil: Distribusi frekuensi polimorfisme gen TGF-β1 (T869C) pada penderita psoriasis paling banyak terdapat pada genotip TC (50,0%) dibandingkan dengan genotip TT (6,5%) dan CC (43,5%). Kesimpulan: varian TC merupakan varian terbanyak dari polimorfisme gen TGF-β1 (T869C) pada penderita psoriasis di kota Medan.Kata kunci: TGF-β1, Psoriasis, Polimorfisme Gen.  Distribution of Beta-1 Transforming Growth Factor Gene Polymorphism (T869C) in Psoriasis Patients in Medan City, North Sumatra  Abstract: Psoriasis is a chronic inflammatory skin disease that is influenced by multifactor with a prevalence in the world around 2-3%. TGF-β1 is a growth factor that has polymorphism in its genes. The TGF-β1 gene polymorphism plays a role in the pathogenesis of psoriasis. Method: This research is a descriptive study with simple random sampling technique. The subject of this research is the stored raw material (BBT) of DNA isolation from 62 psoriasis sufferers consisting of 33 women and 29 men. This study aims to look at the frequency distribution of the TGF-β1 gene polymorphism (T869C) in patients with psoriasis. The TGF-β1 gene polymorphism (T869C) was analyzed by PCR-based restricted fragment length polymorphism (PCR-RFLP). Results: Frequency distribution of TGF-β1 (T869C) gene polymorphism in psoriasis patients was most prevalent in TC genotype (50.0%) compared to TT genotype (6.5%) and CC (43.5%). Conclusion: TC variant is the most variant of TGF-β1 (T869C) gene polymorphism in psoriasis sufferers in Medan city.Keywords: TGF-β1, Psoriasis, Gen Polymorphism.

Abstract

AbstrakPendekatan terbaru dalam teknik rekayasa jaringan adalah penggunaan organ asal yang telah dideselularisasi sebagai scaffold (rangka) untuk rekonstruksi organ. Scaffold yang terbuat dari matriks alami ataupun sintetis memiliki banyak keterbatasan dalam memberikan mikrostruktur matriks ekstraseluler yang ideal untuk proliferasi dan diferensiasi sel. Scaffold asal yang dideselularisasi dapat mempertahankan matriks ekstraseluler organ alami, sehingga merupakan scaffold dengan mikrostruktur yang ideal untuk proliferasi dan diferensiasi sel. Teknik ini menjawab tantangan untuk memenuhi tingginya kebutuhan model hati untuk uji obat. Hati merupakan organ yang kompleks, sehingga dibutuhkan mikrostruktur yang ideal dalam teknik rekonstruksi oragnoid hati untuk menghasilkan fungsi sintetis dan metabolik hati yang baik. AbstractThe latest approach in tissue engineering techniques is the use of deselularized native organ as scaffolds for organ reconstruction. Scaffolds made from natural or synthetic matrix have many shortcomings in providing ideal extracellular matrix microstructure for cell proliferation and differentiation. Deselularized native scaffold can maintain the extracellular matrix of native organs, as an ideal microstructure scaffold for cell proliferation and differentiation. This technique answers the challenge of high need for liver transplants due to the limitations of liver donors and the need as liver models for drug testing.for drug testing. Liver is a complex organ, so ideal microstructure is needed in liver organoid reconstruction techniques to produce good synthetic and metabolic functions of the liver.

Abstract

Intussusceptions, defined as insertion of proximal part of intestine into distal one, it is the second most common acute abdomen pathology after appendicitis. Acute intussusception is one of the common causes of acute intestinal obstruction in children, but on seldom occasions associated with leading points to the intussusceptions. The symptoms are the result of intussusception, like intestinal obstruction, volvulus due to subserosa lipoma and hemorrhage due to ulcerations of the overlying mucosa caused by direct pressure from the lipoma or the intussusception itself. Intussusception in adults is very rare. Most intussusception cases must be treated surgically. In patients who present with a vague history and constellation of abdominal symptoms pointing towards obstruction and are found to have a short segment intussusception, it is a valid option to apply a conservative approach with caution. Based on the available literature, CT scan evaluation seems to have both diagnostic and prognostic value, as it provides enough evidence with regards to signs indicating the need for surgery. Abstract

Pituitary tumors are benign tumors that occur in the pituitary gland with a presentation of 10% of all intracranial tumors. If the size of the tumor is           > 1 cm, the pituitary tumor is called a macroadenoma, the large size will have a mass effect so that there is an emphasis on the tumor area and its surroundings. This emphasis will manifest in hyposecretion of the hormones produced in the pituitary. We report a case of  57-year-old male with complaints of decreased vision since 3.5 months. The patient had previously been diagnosed with a pituitary tumor 6 years ago and had surgery. After diagnostic steps, the patient is diagnosed with a residual pituitary adenoma tumor with hypocortisol and hypothyroidism. Hormone replacement therapy was performed by administering methilprednisolone tablets 16 mg-0-8 mg and levothyroxine tablets 1 x 50 µg. The patient was planned to undergo an Endoscopic Endonasal Excision Tumor surgery with  125 mg/12 hour preoperative injection of methilprednisolon. The provision of preoperative hormone replacement therapy aims to correct hormonal imbalances that can be an intraoperative complication

Abstract

The effect of physical activity on biological functions can be either positive or negative influences. Severe physical activity can increase oxygen consumption by 100-200 times compared to resting conditions. Increased use of oxygen, especially by muscle contracting, causes an increase in the number of free radicals. This study aims to determine the effect of maximum physical activity on cholesterol levels in white rats. The research design was used an experimental study with a post-test and control group design. Subjects were male white rats (Rattus Norvegicus), 20 of them were 3-4 months old, body weight was 180-200 grams. Subjects were divided into 2 groups randomly, each group consisted of 10 white rats, namely the control group: not given maximum physical activity; group treated: given maximum physical activity in the form of swimming until almost submerged. The results showed a significant increase in cholesterol levels (p=0.035; p<0.05) in the treatment group, namely the group that was given maximum physical activity in the form of swimming to almost drowning, compared to the control group, namely the group that did not swim. The study concluded that there was an effect of maximum physical activity on blood cholesterol levels in white rats (Rattus Norvegicus).

Abstract

The nutritional status of patients is one factor that influences the length of stay. This study is an analytic study with a cross-sectional design aiming to observe nutritional status, length of treatment, and the relationship between the two variables. The population in this study were patients aged 19-64 years who were treated in the Internal Medicine KSM at USU Hospital in the period August to October 2019. 52 patients were the study samples, with consecutive sampling as the sample selection technique used. We collected research data as nutritional status and length of stay through anthropometric measurements and structured interviews. We distinguish nutritional status based on Body Mass Index (BMI) and Subjective Global Assessment (SGA). The results showed 34 patients (65.4%) based on BMI values decreased nutritional status from 21.47±3.79 to 21.18±3.83 Kg /m2. Based on SGA, 32 patients (61.5%) experienced a decrease in score. The mean treatment period was 5.02±1.36 days. From the chi-square test results, it is known that changes in nutritional status based on BMI have a significant relationship to the length of stay with a p-value <0.05.

Abstract

Psoriasis is a chronic autoimmune skin disorder that is an autoimmune disease and still considers a global problem. This condition is suspected to be contributed by genetic and environmental factors. Several studies have shown an association between vitamin D gene receptor polymorphisms and the risk of psoriasis. This study was an observational study, which involved 64 research subjects, who were psoriasis patients. Each subject was interviewed and went through physical examination, also Polymerase Chain Reaction - Restriction Fragment Length Polymorphism (PCR-RFLP) examination to determine ApaI (rs7975232) vitamin D receptor gene polymorphism. After the tabulation of the data, we found that the age of psoriasis onset in this study was 21 to 40 years old with an average of 34.40 ± 14.98 years old. The majority of our subjects ApaI VDR gene polymorphism genotype was aa (53.1%). This is consistent with the results of several studies conducted in Asia.

Abstract

Diabetes Mellitus (DM) type 2 is a type of DM with the most sufferers in Indonesia and the world. One of the factors that can affect blood glucose control for people with type 2 diabetes is sleep disturbance. Poor sleep quality causes abnormal glucose tolerance, insulin resistance, and reduced response to insulin, which increases HbA1C levels. The purpose of this study was to determine the relationship between sleep quality and HbA1C levels in type 2 DM patients. This study used an analytic correlation design with a cross-sectional study design. 47 respondents were selected by purposive sampling technique at Dr. Pirngadi Regional General Hospital Medan in 2019. We conducted this research from December 2019 to February 2020. Respondent data was obtained through interviews with the Pittsburgh Sleep Quality Index (PSQI) we obtained a questionnaire and HbA1C levels from the results of the respondents' HbA1C examination in the last three months, which can be seen in the patient's status. We carried data analysis using the SPSS version 21 program out univariate and bivariate using the Spearman test with a confidence level of 95% (α = 0.05) showed a p-value = 0.003 <0.05 and a value of r = 0.423. So that a significant relationship was obtained with a weak correlation between sleep quality and HbA1C levels.
